Enrollment Steps
Thank you for your interest in Candelaria Institute. Follow the steps below to complete your enrollment. Ensure that all requirements are submitted as outlined.
Step 1: Fill Out Forms & Attach Requirements
Fill out the enrollment form and attach the requirements.
Step 2: Pay Enrollment and SSC Membership Fees
Proceed to the cashier to settle the enrollment fee and SSC membership fee.
Step 3: Submit Forms and Requirements
Return to the Information Desk to submit your completed enrollment forms and attached requirements.
Step 4: Entrance Exam
Present your admission slip and take the entrance exam at the Guidance Office.

Old Students

Incoming Grade 7
- Form 138 (Report Card)
- Certificate of Good Moral Character
- PSA Birth Certificate (Photocopy)
Incoming Grade 11
- Form 138 (Report Card)
- Certificate of Good Moral Character
- PSA Birth Certificate (Photocopy)
- ESC Scholarship Certificate (if ESC Scholarship Grantee)

Incoming College Students
- Form 138 (Report Card)

- 1 Long Brown Envelope

Transferred-In Students
